
Heart of Texas HALO exists to make navigating care simpler, more human, and more hope-filled.
We are a community-based care navigation organization serving individuals, families, and referral partners across Central Texas. When someone is facing a health change, crisis, or life transition, the system can feel overwhelming. HALO steps in as a trusted guide — helping connect people to the right level of care, at the right time, with dignity, compassion, and choice at the center of every decision.
Our team works alongside hospitals, physicians, senior care facilities, educators, churches, and health care providers for all levels of care to reduce unnecessary re-hospitalizations, streamline care coordination, and improve quality of life. By handling the time-intensive conversations, resource identification, and follow-up, HALO helps free up providers to focus on what they do best — caring for our aging loved ones.
What sets HALO apart is our patient-choice philosophy. While we collaborate closely with a wide network of trusted providers and services, our role is never to steer — it’s to educate, advocate, and empower. Every recommendation is personalized, unbiased, and rooted in the individual’s goals, values, and needs.

The Community Partnership Program recognizes providers who stand behind our mission: ensuring every senior and family has access to free care navigation, advocacy, education and support - no matter their situation, diagnosis or income.
Our Community Partners are organizations and professionals who believe in coordinated care, informed families and a stronger continuum of support. By recommending HALO's free services and collaborating with our nonprofit, partners help close gaps in care, prevent crisis situations, emergency room visits, re-hospitalizations and bring dignity-centered guidance to the people they serve.

Heart of Texas HALO was built in collaboration with a group of founding community partners who share our commitment to compassionate, coordinated, and patient-centered care. These trusted organizations helped shape HALO’s mission and early outreach, ensuring our services reflect the real needs of individuals, families, and providers across Central Texas.
Our founding partners include Ascension Living Providence Village, Ascension at Home (home health), Bluebonnet Healthcare (home health and hospice), Encompass Inpatient Rehabilitation, and Visiting Angels (independent care provider). While HALO maintains a broad network of community resources and always leads with patient choice, we are grateful for the collaboration, insight, and shared values these partners bring to strengthening care transitions and improving quality of life in our communities.
