Sudden Confusion, Disorientation or Inability to Stay Awake

New Onset of Slurred Speech, Facial Drooping or Weakness on One Side of the Body (Possible Stroke)

Chest Pain, Difficulty Breathing or Blue/Gray Skin Color

A Fall Resulting in Head Injury, Loss of Consciousness, Severe Pain or Inability to Move

New Severe Agitation, Hallucinations or Violent Behaviors that put Themselves or Others at Risk

Seizures or Shaking Episodes that Do Not Stop

Any Rapid Decline that is Unusual
